29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) On or about January 27, 2016, and at times prior, the employees were exposed to electrical shock while employees worked around electrical fixtures and junction boxes that did not have covers. 1) Employees were not protected from electrical shock in that an electrical fixture inside the walk-in freezer door was missing a cover. 2) Employees were not protected from electrical shock in that a junction box underneath the dishwasher station table was missing a cover.

29 CFR 1910.303(b)(6): Conductors or equipment were located in damp or wet locations; were exposed to gases, fumes, vapors, liquids, or other agents that have a deteriorating effect on the conductors or equipment; or were exposed to excessive temperatures: a) On or about January 27, 2016, and times prior, employees were exposed to electrical hazards, where the conductors in the bottom of the cooling unit in the freezer were enclosed in ice and therefore subjected to deterioration and corrosion. b) On or about January 27, 2016, and times prior, the junction box underneath the dishwasher table was exposed to corrosion from moisture.
