29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:         a) On January 28, 2016, employees were exposed to eye injury while operating nail guns and electric saws without eye protection.        In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):            a) On January 28, 2016, employees were exposed to the hazard of falling while performing carpentry work at an elevation of about 22 feet without fall protection.     b) On January 28, 2016, employees were exposed to the hazard of falling from second story window openings where fall protection was not provided.            In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:            a) Second Story Hallway - On January 28, 2016, an 8 foot A-Frame stepladder was provided for employee use for access and egress between two points of the building.          No abatement certification or documentation required for this violation.
