29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  On or about January 26, 2016 at the worksite located at 25166 E. 2200 Rd., LaCygne, Kansas, the employer failed to ensure that components of the personal fall arrest system, such as but not limited to the self-retracting lifeline and anchorage were inspected prior to each use.  An employee engaged in demolition work was exposed to fall hazards of approximately 30 ft. 5 inches from the working level to the ground.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  On or about January 26, 2016 at the worksite located at 25166 E. 2200 Rd., LaCygne, Kansas, an employee engaged in demolition work was exposed to fall hazards of approximately 30 ft. 5 inches from the working level to the ground in that the anchorage used did not meet the specifications required by the standard.
