29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Coast Guard housing: On or about January 19th 2016 and times prior thereto, two employees were using a aerial lift for installing siding were not connected by their fall protection lanyard. This exposed the employees to a 10 foot fall hazard to the asphalt below.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment. Coast Guard housing: On or about January 19th 2016 and times prior thereto, two employees in an aerial lift did not have their lanyard connected and did not have their harness worn correctly. These hazards show that the employees need to be retrained on the correct use of personal fall arrest equipment.
