29 CFR 1910.134(c)(1)(vi): The written program did not contain procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators.    On or about January 28, 2016:   The employees working in the spray painting area and the abrasive blasting area are exposed to respiratory hazards wear they are required to wear supplied air respirators such as the Clemco Apollo, and the Allegro 9903 when blasting and painting metal parts, respectively.  The employer did not have written procedures in place or implemented to ensure the air quality of the supplied breathing air to the employees.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.   On or about January 28, 2016: Employees working in the spray bay are required to wear air purifying respirators (3M 6000 Series) and supplied air respirators (Allegro 9903 and Clemco Apollo 80).  The back-up painter and blaster did not receive a medical evaluation prior to their initial use of these respirators.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:   On or about January 28, 2016: Employees working in the spray paint area are exposed to hazards such as but not limited to isocyanates and are required to wear a respirator.  The spray painters required to wear tight fitting respirators were not fit tested prior to their use of the 3M 6200 and 6800 Respirators.

On or about January 28, 2016: Employees working in the facility located at 10 Fesco Way are exposed to chemicals such as but not limited to isocyanates.  The employees were not trained on the specific chemical hazards and symptoms associated with those hazards.
