29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   (a) Jobsite:  On or about January, 27, 2016, and employee was not wearing a hard hat while he was working below one employee working from an aerial lift and one employee working from the roof.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:    (a) Jobsite:  On or about January 27, 2015, an employee was working from a JLG aerial lift and he was not tied off.  The fall hazard to the employee was up to about 33 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) Jobsite:  On or about January 27, 2016, an employee was working on a roof and he was not protected from falling.  The pitch of the roof was 6 in 12 and the ground to eave height was about 33 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a) Jobsite:  On or about January 27, 2016, two employees need to be retained on when to use fall protection.  One employee was working from a roof and the second employee was not tied off while working from an aerial lift.
