29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, Fairfield Inn, 2915 Pleasant Valley Boulevard, Altoona, PA: On or about January 28, 2016 employees who were laying plywood flooring and constructing a leading edge were not protected from falling approximately 14'4" to the level below. There were no guardrail systems, safety net systems or personal fall arrest systems in use to protect the employee from falls.
