29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights    a) On or about January 6, 2016; south-side of residential home; the working platform on the two-point suspension ladder was not fully planked exposing employees to a 5-15 foot fall to the lower roof area; the platform missing one plank.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:  a) On or about January 6, 2016; south-side of residential home; employee working off a two-point suspension scaffold without a guard rail system installed to help protect the employee from an estimated 5-15 foot fall to the lower levels while completing his assigned duties.
