29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: a) Worksite, 600 Merchants Concourse, Westbury, NY- Employee was working on a walkway with a missing piece of grating thus creating an unprotected floor hole measuring approximately 3'x2' on the grating of an overhead bridge crane; on or about 1/20/2016. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
