29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   Instances including, but not limited to the following:  a) Job site, 5100 Campbells Run Road, Pittsburgh, PA: On or about March 22, 2016, during construction activities, the employer did not ensure that the scaffold system in use was given an appropriate safety inspection by a competent person prior to the start of work. Deficiencies noted during the inspection included, but were not limited to, the lack of fall protection systems/devices being used by employees performing work on the scaffold and the lack of a safe means of moving between two scaffold systems.  b) Job site, 5100 Campbells Run Road, Pittsburgh, PA: On or about April 1, 2016, during construction activities, the employer did not ensure that the scaffold system in use was given an appropriate safety inspection by a competent person prior to the start of work. Deficiencies noted during the inspection included, but were not limited to, the lack of pins being used in frame and post panels and failing to provide adequate planking on all working areas.
