29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  a) Workplace: The employer provided documentation that a hazard assessment had been conducted at the worksite. The form failed to fully identify the specific areas in which an assessment was conducted; There is no name of a person who certified that the evaluation had been performed; and no specific dates of the hazard assessment were provided.  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Worksite: The employer did not ensure that employees engaged in the voluntary use of a Gerson 1730 N95 filtering face piece type respirators while working in Detyens Shipyard were provide with the information in Appendix D.
