29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person: a) Job site, 5100 Campbells Runs Road, Pittsburgh, PA: On or about April 1, 2016, the employer did not ensure that appropriate safety and health inspections were conducted by a competent person at the worksite. Deficiencies noted during the inspection included, but were not limited to, lack of hand rails on stairs, an electrical box hung from the ceiling by it's romex wiring, floor holes in a hallway, contractors not following proper energy control protocols while working on electrical systems, workers not using eye/face protection while sanding walls/ceilings where dust was dropping into their faces, contractors working on scaffolding that was not properly set up, cords laying in the walking/working areas, missing face plates on live electrical components such as outlets, industrial fans with guards damaged and using extension cords plugged into one another. b) Job site, 5100 Campbells Runs Road, Pittsburgh, PA: On or about March 22, 2016, the employer did not ensure that appropriate safety and health inspections were conducted by a competent person at the worksite. Deficiencies noted during the inspection included, but were not limited to, failing to ensure that appropriate fall protection systems/devices were used on scaffolding and failing to ensure that a safe way to move between two scaffolds existed. c) Job site, 5100 Campbells Runs Road, Pittsburgh, PA: On or about January 27, 2016, the employer did not ensure that appropriate safety and health inspections were conducted by a competent person at the worksite. Deficiencies noted during the inspection included, but were not limited to, failing to ensure that fall protection systems/devices were used on roofs, scaffolding or in aerial lifts.
