29 CFR 1926.152(d)(2): A portable fire extinguisher having a rating of not less than 20-B units was not located between 25 feet and 75 feet from flammable liquid storage area(s) located outside:  On or about December 10, 2015, at 1061 Collins Hill Road, Lawrenceville, Georgia:  Several employees performing construction work were exposed to burn hazards while working near an approximately 500 gallon diesel tank used to refuel motorized equipment when the employer did not provide a fire extinguisher.  The diesel fuel is a class 3 flammable.

29 CFR 1926.152(g)(9): Conspicuous and legible signs prohibiting smoking were not posted in service and refueling areas:  On or about December 10, 2015, at 1061 Collins Hill Road, Lawrenceville, Georgia:  Several employees performing construction work were exposed to burn hazards while working near an approximately 500 gallon diesel fuel tank used to refuel motorized equipment when the employer did not post a "No Smoking" sign in the refueling area.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  On or about December 10, 2015, at 1061 Collins Hill Road, Lawrenceville, Georgia:  An employee performing block masonry work was exposed to a fall hazard of approximately 20-feet and greater when he worked from a wooden pallet held up by a JLG Sky Trak Telehandler 8042 without using fall protection.
