29 CFR 1926.651(d): Exposure to vehicular traffic. Employees exposed to public vehicular traffic shall be provided with, and shall wear, warning vests or other suitable garments marked with or made of reflectorized or high-visibility material.  a. At the worksite where employees were working adjacent to and on the roadway, reflective vests or other suitable garments were not used, thereby exposing the employees to a struck-by hazard.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:   a. At the worksite where employees were installing a water main disconnect and a T-valve in a trench, cave-in protection was not provided by an adequate protective system, thereby exposing the employees to a crushed-by hazard.
