29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     The employer is failing to protect employees from fall hazards associated with working from a scaffold platform that was approximately 14 feet to the ground below.  Employees were using a tubular framed scaffold which was not fully decked or planked at all working levels.  This exposes the employees to potential fall hazards.

29 CFR 1926.451(e)(1): Cross braces were used as a means of access:     The employer is failing to protect employees from potential fall hazards associated with accessing a scaffold platform that is 14 feet above the ground below.  Employees were using cross bracing to climb directly up the side of the scaffold while installing glass panels.  This exposes the employees to potential fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     The employer is failing to protect employees from a fall hazard of approximately 14 feet to the ground level below while the employees were installing glass panels.  Employees were working on a platform being elevated in the air approximately 14 feet without any form of fall protection including, but not limited to, guard rail systems, safety nets or personal fall arrest systems.

Employees were conducting window installation on a building from a tubular frame scaffold without fall protection.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:     The employer is failing to ensure employees who are erecting, disassembling, moving, operating, repairing, maintaining or inspecting a scaffold are trained by a competent person to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Employees were conducting window installation on a building from a tubular frame scaffold without fall protection.  This scaffold also had hazards including, but not limited to, no access ladder, working levels were not fully planked and no guardrails.
