29 CFR 1910.178(l)(4)(ii): Refresher training in relevant topics shall be provided to the operator when:  29 CFR 1910.178(l)(4)(ii)(B): The operator has been involved in an accident or near-miss incident;  a.  On or about 5-8-2015 a powered industrial truck operator was involved in an accident when traveling around a corner and struck another employee in the aisle and the employee was not retrained before allowing the operator to continue to operate the forklift.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.  a.  In the production area, the back panel of the Buhr Machine was removed where employees went to gather bags and rolls of plastic thereby exposing employees to a caught-in hazard.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  a.  In the Bindery Area where books are bound, there were stacks of crates and trash can stored in front of the electrical panel PS-2 thereby exposing employees to an electrical panel.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  a.  In the production area behind the Buhr Machine, there was an extension cord pugged in to the electrical outlet which did not have a ground prong thereby exposing employees to an electrical hazard.
