29 CFR 1910.36(g)(2): The exit access route was not at least 28 inches wide (71.1 cm) wide at all points. a/Advance Machine, Inc./Linden, NJ Site: Northeast corner of the shop. A minimum width of 28 inches of the exit access route was not maintained at the employees work station the aisle near the Engine lathe is blocked by raw metal stock. Observed on or about 01/29/2016. b) Advance Machine, Inc./Linden, NJ Site: milling shop area. A minimum width of 28 inches of the exit access route was not maintained. Raw metal stock were stored in the exit access route exposing employees to tripping hazards while exiting the shop during an emergency. Requiring employees to step over the product. Observed on or about 07/25/2016

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a/ Advanced Machine Inc.,/ Linden, NJ -In the Northwest corner of the shop area, The employer did not ensure adequate guarding was in place and it's use enforced. The guard provided allows employee's to operate equipment without the guard in place. An employee sustained fatal injuries while operating without the guard in place. Observed on 1/29/2016
