29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: (a) Adjacent to the Bandsaw: On or about 1/19/2016, on the floor at the feed side of the band saw, both the anti-fatigue mat and the floor surrounding the mat was not maintained in a clean and dry condition, in that the overspray of the Camie 888 Silicone Mold Release had built up on both the mat and the floor creating a slipping hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Cardboard Cutting Area: On or about 1/20/2016, the sliding guard of the bandsaw blade was not adequate in that a hole had been cut from the top front side exposing the rotating saw blade.
