29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: A) International Automotive Components Group, Inc., located at 555 West Linfoot Street, Wauseon, OH 43567: On or about January 7, 2016, the employer did not ensure that gravitational energy was relieved or restrained while employees were installing or removing the T300 Instrument Panel Die # 5490 (weight 58,200 lbs.) in the Ferromatik Milacron injection press # 2 (Model ML 3500-362; Serial # H40A0100005). Employees were exposed to crushing hazards.
