On or about 1/29/16 in the central part of the building; the employer allowed employees to walk and operate the Clark propane fork truck (Model # GPS25MC) in the aisle near a broken floor 4' by 2' in diameter where the fork truck comes within 2' of the broken floor; thus exposing employees to potential struck-by and foot injury.
