OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:  Employees engaged in material handling activities in the Fab Shop by the big machines area were exposed to struck-by hazards in that hooks on the 10-Ton R&M overhead cranes were not provided with safety latches.  Among other means, one feasible and acceptable methods to abate this hazard is to install safety latches on the hooks as required by Section 2-1.14.5, Hooks, of ASME 30.2-2001.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:  Employees engaged in grinding activities at the blue Baldor bench grinder in the middle of the Fab Shop by the tool storage area were exposed to struck-by hazards in that the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1.5 inches on the right side.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near another substance likely to cause or accelerate fire; or in an acetylene generator compartment:  Employees engaged in daily activities in the Fab Shop by the entrance door and overhead door were exposed to fire hazards in that the full Oxygen and Acetylene cylinders were stored together.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1910.184(e)(3)(ii): The employer did not make and maintain a record of the most recent month in which each alloy steel chain sling was thoroughly inspected, and did not make such record available for examination:  A record of the most recent periodic inspection of the alloy steel chain sling was not maintained for the 10-Ton R&M cranes.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
