29 CFR 1910.303(b)(7): Electric equipment was not installed in a neat and workmanlike manner: a) on or about January 27, 2016 and times prior thereto, Suburban Tool & Die Co., Inc. in Erie, PA. The wiring of the Daikin Inverters, behind the graphite CNC machines, was loose and the inner wires were exposed. Conduit, or other means, was not utilized to protect the wires coming out of the inverters to the electrical power source.

On or about January 27, 2016 and times prior thereto, extension cords were used in lieu of permanent wiring to supply power to computer equipment at the workstation. b) Suburban Tool & Die, cylindrical grinders. On or about January 27, 2016, and times prior thereto, an extension cord was used in lieu of permanent wiring to supply power to the work lamp on the grinder. c) Suburban Tool & Die, cylindrical grinders. On or about January 27, 2016, and times prior thereto, the flexible cord for the work lamp was concealed under the floor.

29 CFR 1910.145(c)(3): Safety instruction signs were not used where there is a need for general instructions and suggestions relative to safety measures: a) cylindrical grinder, Suburban Tool & Die Co., Inc. in Erie, PA. On or about January 27, 2016, warning stickers were not affixed to the grinder to alert employees of the caught-by hazards created by the rotating assembly that contains the grinding wheels and the stationary framework of the machine.
