29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about January 27, 2016 and times prior; Phoenix S000607 horizontal band saw: The employer failed to ensure the unused portion of the blade was guarded to prevent inadvertent contact during use.  b) On or about January 27, 2016 and times prior; Phoenix D02066 horizontal band saw: The employer failed to ensure the unused portion of the blade was guarded to prevent inadvertent contact during use.  c) On or about January 27, 2016 and times prior; big lathe: The employer failed to ensure the lathes were properly guarded to prevent inadvertent contact with the point of operation during operation and metal shavings from hitting the employee.  d) On or about January 27, 2016 and times prior; Atrump milling machine: The employer failed to ensure the milling machines were properly guarded to prevent inadvertent contact with the rotating bit during operation and metal shavings from striking the employee.
