On or about February 2, 2015, while installing masonry materials on a newly constructed residential home located at 22348 Pinnacle Point, Strongsville, Ohio 44136, the employer failed to protect employees from a fall of approximately 15 feet by fully planking all working levels of a supported scaffold.
