29 CFR 1926.502(d)(16)(ii): Personal Fall Arrest Systems, when stopping a fall, did not limit maximum arresting force on an employee to 1,800 pounds (8 kN) when used with a body harness:  Instance 1:  On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 6' shock absorbing lanyard and attached to another web sling 6' lanyard increasing his fall distance which potentially increases the arresting force during a fall.   Instance 2:  On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 9' self-retractable lanyard and attached to a 6' cable that was attached to an anchor embedded in the concrete slab increasing his fall distance which potentially increases the arresting force during a fall.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Instance 1:  On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 6' shock absorbing lanyard and attached to another web sling 6' lanyard increasing the free fall distance more than 6 feet.    Instance 2:  On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 9' self-retractable lanyard and attached to a 6' cable that was attached to an anchor embedded in the concrete slab increasing the free fall distance more than 6 feet.

29 CFR 1926.502(e)(8)(i):  Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged directly to webbing, rope or wire rope:  On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 6' shock absorbing lanyard, which the snaphook was engaged directly to the webbing of a 6' web sling lanyard.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  On or about January 7, 2016, at the above addressed job site, employees using a self-retracting lanyard incorrectly while laying blocks on the 2nd floor of a residential townhouse were not trained by a competent person in the use and operation of a fall protection system.
