29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     On or about February 1, 2016, during the installation of windows on a new construction single family home at 7408 Galena Ave NW, Canal Fulton, Ohio 44614; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces with a height of approximately 11.11 feet above the ground.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     On or about February 1, 2016 during the structural framing and installation of windows at the job site located at 7408 Galena Ave NW, Canal Fulton, Ohio 44614; the employer did not ensure employees working on the roof of a new construction single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing windows on the second floor roof.
