29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about January 22, 2016, the employer failed to ensure that the cutting blade portion of the Komax 206 slicing machine (ser. no. 206.0096) was effectively guarded to prevent employee exposure to the point of operation. The exit side chute which acts as the guard was not in place during operation.
