29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     On or about February 1, 2016, during the installation of windows on a new construction single family home at 900 Braemar Cir NE, N. Canton, Ohio 44720; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces with a height of approximately 21 feet above the ground.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     On or about February 2, 2016 during the structural framing and truss installation at the job site located at 900 Braemar Cir NE, N. Canton, Ohio 44720; the employer did not ensure employees working on the roof of a new construction single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing framing material with an air driven framing nail gun while working at height on the roof.
