29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)  Fabrication Shop;  On or about January 21, 2016 and at times prior thereto, the employer failed to develop machine specific lock out tag out procedures for the Fadal 6030 milling machine which has electrical and pneumatic energies present.  b)  Fabrication Shop;  On or about January 21, 2016 and at times prior thereto, the employer failed to develop machine specific lock out tag out procedures for the Fadal 6535 milling machine which has electrical and pneumatic energies present.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a)  Fabrication Shop;  On or about January 21, 2016 and at times prior thereto, the employer failed to provide employees who voluntarily use half mask respirators and filtering face-piece respirators when grinding with the information contained in Appendix D of the Respiratory Protection standard.
