29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) In and around the building under construction, on or about 02/25/2016, employees who were not wearing hard hats were observed performing framing work and walking below the area where carpentry activities were conducted, that presented a head injury hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) In the back of the building under construction, on or about 02/25/2016, employees were observed working and walking on the top plate of the first floor of the building without wearing fall protection, exposed to a fall of more than nine feet (9') onto the ground below. b) Inside the building under construction, on or about 02/25/2016, employees were observed working and walking on a horizontal ceiling brace without wearing fall protection, exposed to a fall of more than sixteen feet (16') onto the cement floor below.
