29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. West wall in Binding Area - on or about February 25, 2016 employees operated a three hole paper drill manufactured by Challenge to punch holes in stacks of paper. The point of operation was not guarded, exposing employees to caught-in hazards.    b. West wall in Binding Area - on or about February 25, 2016 employees operated a single hole paper drill with model number 4264 manufactured by Challenge to punch holes in stacks of paper. The point of operation was not guarded, exposing employees to caught-in hazards.    c. Press Room - on or about February 25, 2016, employees operated an Original Heidelberg windmill press used for die cutting operations. Machine guarding was not provided to protect operators and other employees from the ingoing nip points at the ink rollers.    d. Press Room - on or about February 25, 2016, employees operated an Original Heidelberg windmill press used for die cutting operations. Machine guarding was not provided to protect operators and other employees from the rotating windmill, exposing employees to struck-by hazards.    e. Press Room - on or about February 25, 2016, employees operated an Original Heidelberg windmill press used for die cutting operations. Machine guarding was not provided to protect operators and other employees from the platen zone, exposing employees to caught-in hazards.
