1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet was not guarded;     (a) RLW Sand Storage, LLC. at 4390 Madison Street in Denver, CO 80216: On and before January 27, 2016,  the employer did not ensure that the wall opening on the 8th floor was equipped with a guard rail, roller, picket fence, half door, or equivalent barrier.  The employees opened up doors inward to expose the wall opening on a regular basis for ventilation.  The wall opening measured approximately 4 feet x 7 feet.  This condition exposed employees to fall hazards of approximately 70 feet.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:    a)  RLW Sand Storage, LLC. at 4390 Madison Street in Denver, CO 80216: On and before January 27, 2016, the employer did not ensure that an open-sided platform over 4 feet tall was equipped with a standard railing on open sides. The fixed platform is used to cross over a conveyor belt used to transport sand to the north and south tripper. The employer is required to provide protection from falls where elevated walkways are in use. The condition exposed employees to falls of approximately 5 feet.

29 CFR 1910.147(c)(4)(i):   Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) RLW Sand Storage, LLC. at 4390 Madison Street in Denver, CO 80216:  On and before January 27, 2016, machine specific, step-by-step procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees performed maintenance.  This condition exposed employees to hazards resulting from electrical, pneumatic, kinetic, and gravity energy sources.  Maintenance was performed on equipment such as, but not limited to:     (1)     Bucket Elevators (aka. Bucket Legs) on the East and West sides; and  (2)     Conveyor Belts used to transport sand to the North and South Trippers.     Condition exposed employees to amputation hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a) RLW Sand Storage, LLC. at 4390 Madison Street in Denver, CO 80216: On and before January 11, 2016 the employer did not conduct a periodic inspection at least annually of the Lockout/Tagout (LOTO) procedures used by maintenance personnel. This condition exposed employees to hazards resulting from electrical, pneumatic, kinetic, and gravity energy sources.  Maintenance was performed on equipment such as, but not limited to:     (1)     Bucket Elevators (aka. Bucket Legs) on the East and West sides; and  (2)     Conveyor Belts used to transport sand to the North and South Trippers.     Condition exposed employees to amputation hazards.
