29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems. On February 2, 2015, employees were working on the roof of a commercial building without using any form of fall protection. Employees were exposed to fall hazards of more than six feet to the ground below.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. This condition exposed employees to falls greater than 6 feet.
