Sources of ingition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: At this well site, Outlar #1 employees were exposed to fire and explosion hazards while tasked to cut a casing pipe with a cutting torch and the employer did not ensure flammable gasses were not present.

The employees were exposed to natural gases where a flash fire had occurred.
