Sources of ingition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: At this well site, Outlar #1 employees were exposed to fire and explosion hazards while tasked to cut a casing pipe with a cutting torch and the employer did not ensure flammable gasses were not present.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: At this well-site, Outlar #1, a hazard assessment was not conducted to determine the need for personal protective equipment such as, but not limited to, fire retardant clothing, safety glasses, hand protection, mask, four gas meter for atmospheric testing. The employees were exposed to natural gases where a flash fire had occurred.
