29 CFR 1910.22(a)(1):  In the following instances, all places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:  a)    Mezzanine - Sections of the perforated metal floor were not secured or were not laying flat which created a tripping hazard along the edges. Debris such as cardboard boxes and construction materials were also cluttering the floor.  b)    Throughout the facility, especially around saws without dust collection systems,  an accumulation of wood dust was present creating a fire/explosion hazard.
