29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     This violation was observed on or about January 26, 2016, the employer did not ensure machine guarding was in place on the Pacific hydraulic press brake to protect employees from caught in hazards.       Alfa Laval, Inc. was previously cited for this violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314245945, Citation number 2, item number 1, and was affirmed as a final order on August 22, 2011 with a final abatement date of August 31, 2011, with respect to a workplace hazard at 300 Chestnut Street Lykens, PA 17048.       Alfa Laval, Inc. was previously cited for this violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1012281, Citation number 2, item number 2, group number a, and was affirmed as a final order on December 10, 2015 with a final abatement date of November 1, 2015, with respect to a workplace hazard at 1201 S. 9th St., Broken Arrow, OK  74012.
