Avon, OH: The employer did not ensure employees were wearing appropriate eye protection such as, safety glasses while using a pneumatic nail gun to fasten siding material. The employees using these tools in this condition were exposed to potential eye injuries from flying objects.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another. a) Holmes Siding Contractors on or about February 02, 2016, at the address of 2457 Woodhawk Ln. Avon, OH: The employer failed to ensure the two pick platforms applied to a ladder jack scaffold system were individually supported, preventing unintentional displacement. An employee installing siding material from a bridged platform system was exposed to a fall of 13 ft. to the level below.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold is not protected by a personal fall arrest system: a) Holmes Siding Contractors: The employer does not ensure each employee installing siding on a residential structure and working from a ladder jack scaffold is protected from falls. This violation was most recently observed on February 02, 2016, at the address of 2457 Woodhawk Lane, Avon, Ohio where employees on the north side of the structure were exposed to an approximately 15 feet fall. To abate this violation, the employer must ensure that its employees performing siding work are protected from falls by utilizing one or more fall protection systems. b) Holmes Siding Contractors: The employer does not ensure each employee installing siding on a residential structure and working from a ladder jack scaffold is protected from falls. This violation was most recently observed on February 02, 2016, at the address of 2457 Woodhawk Lane, Avon, Ohio where employees on the west side of the structure were exposed to an approximately 15 feet fall. To abate this violation, the employer must ensure that its employees performing siding work are protected from falls by utilizing one or more fall protection systems.
