29 CFR 1926.403(e): All splices and joints and free ends of conductors were not covered with insulation equivalent to that of the conductors or with an insulating device designed for the purpose.  a) 190 Bowery St, Manhattan, NY, Temporary Lighting circuit. On or about 1/27/16: Wiring used as a temporary lighting circuit contained splices that used black electrical tape as insulation which was not equivalent to the original insulation properties.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. 190 Bowery St, New York, NY  Employees throughout the jobsite were observed engaged in construction activities in areas where flexible cords were used without any means of strain relief exposing employee(s) to electrical shock hazards.
