29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or on about February 3, 2016, at the above addressed site, an employee was exposed to a fall hazard when performing power washing work at the edge of a walking/working surface approximately 11 feet 8 inches in height while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about February 3, 2016, at the above addressed site, an employee performing power washing work exposed to a fall hazard while on a walking/working surface approximately 11 feet 8 inches in height was not provided with a training program for him to recognize the hazards associated with falls.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 3, 2016, at the above addressed site, an employee was exposed to a fall hazard when he gained access to a walking/walking surface approximately 11 feet 8 inches in height using an extension ladder that did not have the side rails extended at least 3 feet past the upper landing surface.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:   On or about February 3, 2016, at the above addressed site, an employee was exposed to a fall hazard when he gained access to a walking/walking surface approximately 11 feet 8 inches in height using an extension ladder that was not secured to prevent accidental displacement while it was set up in an active drive-thru lane.
