29 CFR 1910.178(l)(3)(i)(M): Powered industrial truck operators did not receive initial training on truck-related topics including operating instructions, warnings, or precautions listed in the operators manual for the types of vehicles that the employees were being trained to operate.  8720 Morrisette Dr., Springfield, VA 22152: On or about February 3, 2016, the employer did not train forklift operators on topics listed in the operators manual, exposing employees to struck-by, crushing and other hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  8720 Morrisette Dr., Springfield, VA 22152: On or about February 3, 2016, a warehouse access to an exit door was obstructed by materials such as, but not limited to, computer equipment, exposing employees to crushing and other hazards in the event of an evacuation.  Notice 1, Item 1, 29 CFR 1910.37(a)(3) as required by 29 CFR 1960.8(b): the employer failed to keep exit routes free and unobstructed. The employer was cited for a violation the same standard on March 4, 2015, Inspection number 1037490, March 30, 2015 final order/settlement, and means of abatement.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:  8720 Morrisette Dr., Springfield, VA 22152: On or about February 3, 2016, two warehouse exit doors were each marked by a sign reading "Exit" but were not illuminated, exposing employees to smoke inhalation and other hazards in the event of an evacuation.  Certification and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.
