29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a)  On or about February 3, 2016, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing work such as, but not limited to, blade changes on the table saws.  Failure to lock out the energy sources while these tasks are being performed exposes employees to amputation injuries.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a)    On January 8, 2016, the employer did not assure the Powermatic table saw was guarded.  An employee using the saw to cut stakes suffered an amputation injury during operation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     a)  On January 8, 2016, an employee was operating an unguarded table saw and sustained an amputation injury; the employer did not report the amputation within 24 hours.
