29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.   On the east side of the work site, employees standing on a wooden platform installing cornice material were elevated by a JLG forklift that was not approved by the forklift manufacturer and exposed to fall, struck-by, and crush-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On the east side of the construction site, employees engaged in roofing activities on a pitch roof of 16:12 without fall protection were exposed to a fall hazard of approximately 12 feet to the ground below.
