Failure to provide voluntary respirator users with a copy of Appendix D should be cited under 29 CFR 1910.134(k)(6).  The employer did not provide Appendix D for employees were exposed to health hazards and skin infections while voluntarily using a half mask respirator while mixing and using cleaning chemicals in the kennel, cabin and yard areas.
