OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees operating  powered industrial trucks while working from a platform were exposed to fall hazards:    On or about January 25, 2016, employees were exposed to fall hazards while working from a platform resting on the forks of the forklift because the platform did not provide protection for personnel on the platform from moving parts on the truck that present a hazard, provide a restraining means such as a guardrail or another means of securing personnel, means to securely attach the platform to the lift carriage or forks, and the platform did not indicate the maximum work load including personnel and equipment in accordance with ANSI B56.1-2009, American National Standard for Safety Standard for Low Lift and High Lift Trucks, Section 7.37.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      On or about February 3, 2016, employees in the shop area were exposed to electric shock when using welders that had the strain relief pulled exposing live conductors.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    On or about February 3, 2016, employees were exposed to cleaners, solvents, oils, detergents, and paints and the employer had not provided training that addressed the following information:               1.      The requirement of this section; 2.      Any operation in their work area where hazardous chemicals are present; 3.      The location and availability of the written hazard communication program.    Employee training did not include:    1.    Methods and observations to detect the presence/release of a hazardous chemical; 2.    The physical and health hazards of the chemicals in the work area; 3.    The measures employees can take to protect themselves; 4.    The details of the hazard communication program developed by the employer.
