29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about February 03, 2016, and at times prior thereto, the employees were not trained by a competent person to recognize the hazards associated with the use of a pump jack scaffold system, exposing the employees to fall hazard of approximately 12 feet.

Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage.     On or about February 03, 2016, the employer did not ensure that the pump jack brackets, braces, and accessories were fabricated from metal plates and angles on the scaffolding system installed on the west side of the property under construction, exposing the pump jack scaffold to failure or slippage.
