29 CFR 1910.334(b)(2): After a circuit was de-energized by a circuit protective device, the circuit was manually reenergized without determining that the equipment and circuit can be safely energized.  a)    Portsmouth Marine Terminal: On or about 01/28/2016, the circuit breaker in pit #8 tripped resulting from a fault in the shore power cable. The circuit breaker was reclosed without determining the circuit was safe, an arc flash occurred resulting in facial burns to an employee.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  a)      Portsmouth Marine Terminal, Pit #8: On or about 01/28/2016, an employee was not provided protection from possible electric shock in that rubber insulated gloves were not used in testing exposed 480 volts conductors.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.335(a)(1)(v): Employees exposed to the danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion did not wear protective equipment for the eyes or face:    a)      Portsmouth Marine Terminal, Pit #8: On or about 01/28/2016, employees working with 480 volts were not required to wear a face shield while troubleshooting shore power as a result a 480 volts plug arc flashed causing facial burns to an employee.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
