29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: a) Magnolia Tire Shop, 1048 Magnolia Avenue, Elizabeth, NJ site - Outdoor service area: The employer failed to report an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On or about January 22, 2016, a laborer was injured and lost his eye when the rim of a small "snow blower" tire exploded while he was inflating it with air. OSHA was not notified of the employee's injury by the employer. Condition noted on or about 2/1/2016.
