29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions: a) On February 3, 2016, two employees working inside the bathrooms at the Pearl Harbor Joint Base Gym, were applying a Quikrete product to the floors without wearing gloves; exposing their hands to corrosive hazard.

29 CFR 1926.405(j)(1)(ii): Fixtures, lampholders, rosettes, or receptacles were not securely supported: a) On February 3, 2016, employees working the woman's bathroom at the Joint Base Pearl Harbor Gym, were exposed to an electric shock as a result of the outlet box being used was pulled away from the wall.
