29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as making minor adjustments, and/or when performing preventative maintenance.  (a) On or about June 4, 2016, the employer did not ensure that energy control procedures were utilized for the control of hazardous energy when performing servicing and maintenance, which included changing the bearings on the RAP 7020 ARPAC wrapper/heat tunnel, exposing employees to the hazards of pneumatic, electrical and thermal energy.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage, and removal of energy controls were acquired by employees.  (a) On or about June 4, 2016, the employer did not ensure all employees that performed servicing and maintenance on the RAP 7020 Arapac Wrapper/Heat Tunnel System were adequately trained to acquire the knowledge and skills required for the safe application, usage, and removal of the energy controls.  Employees were "Authorized" without knowing the magnitude of the energy available in the workplace and the methods and means necessary for energy isolation and control.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) Maintenance Shop - On February 11, 2016, the exit door did not open freely from the inside due to damage to the door.    Aryzta, LLC, was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.36(d)(1), which was contained in OSHA inspection number 1098353, citation number 1, item number 1, and was affirmed as a final order on January 20, 2015, with respect to the workplace located at 401 E. Joe Orr Road, Chicago Heights, Illinois.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

(a) On or about February 3, 2016, the employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that each procedure and the requirements of the standard are being followed when employees are exposed to potentially hazardous energy.   Aryzta, LLC, was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.147(c)(6)(i) which was contained in OSHA inspection number 1020742, citation number 2, item number 1, and was affirmed as a final order on April 30, 2015, with respect to the workplace located at 300 Innovation Drive, Romeoville, Illinois.    Aryzta, LLC, was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.147(c)(6)(i) which was contained in OSHA inspection number 920696, citation number 1, item number 3 and was affirmed as a final order on October 23, 2013, with respect to the workplace located at 64 Chester Street, Rochester, New York.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
