29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about February 4, 2016, employees were exposed to pinch points created by the lift table on an Automate Industrial Tech M2L deckboard stacking machine located in the cutting room. (b) On or about February 4, 2016, employees were exposed to the ingoing pinch point created by the conveyor feeding the McDONOUGH Manufacturing Company Band Mill.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: (a) On or about February 4, 2016, conductors entering a subpanel to the hydraulic pump powering the McDONOUGH Manufacturing Company Band Mill were not protected against abrasion as the flexible conduit fitting was not effectively closed to the box. (b) On or about February 4, 2016, conductors entering the Automated Industrial Tech M2L deck board stacker were not protected against abrasion as the flexible conduit fitting was not effectively closed to the machine.
